Differentiate SCAPE from cardiogenic shock pulmonary edema by assessing blood pressure, perfusion, and the underlying hemodynamic phenotype rather than treating all pulmonary edema identically.

Explain how CPAP and BiPAP affect cardiopulmonary physiology, including alveolar recruitment, reduced work of breathing, decreased venous return, and reduced left-ventricular afterload.

Select and reassess treatment based on physiology, using early NIV and vasodilation for hypertensive pulmonary edema while applying positive pressure cautiously, supporting perfusion, and recognizing early NIV failure in cardiogenic shock.

About the course

This course examines the use of noninvasive positive-pressure ventilation in acute cardiogenic pulmonary edema, emphasizing the critical distinction between hypertensive pulmonary edema—particularly SCAPE—and pulmonary edema associated with hypotension or cardiogenic shock. Through physiologic review and clinical cases, learners explore how CPAP and BiPAP improve oxygenation, reduce work of breathing, and alter ventricular preload and afterload; select and titrate appropriate initial settings; integrate vasodilator or circulatory-support strategies according to the patient’s hemodynamic phenotype; and recognize early indicators of NIV failure requiring escalation to invasive ventilation.
